Asthma information

Every student with a diagnosis of Asthma documented on their physical examination MUST submit an Asthma Action Plan annually along with the necessary medications by the 1st day of school.

All medications must be kept in the health office.  If your child uses an inhaler, please contact the school nurse.  Please complete, with your child's doctor, the asthma action plan that can be found on the link below.  With signatures, this plan serves as both a MD order and a parent permission form.  Please sign the form in the appropriate places, and have your child's MD complete the order, and sign the form as well.
